Step 2: Under “I am a new user, how do I register?” On the bottom part click on “here” highlighted in color blue.

Step 3: You will be routed to Chambers of Commerce and Industry Electronic Certificates of Origins on Annex (by Letters), you may click the specific document format that you need.

NOTE: You may also go to your respective Chambers website to have the format.
